- Two preventive hygiene visits per year (professional cleaning and polish)

- Oral cancer screening at every exam
- One emergency exam for urgent dental issues during your plan year
- 10% discount on all treatment completed at our practice (excluding Invisalign and teeth whitening)
Periodontal
$645/Year
Includes:
- Three preventive hygiene/perio maintenance visits per year (based on Dr. recommendation)
- Two doctor exams (comprehensive and periodic)
- Routine annual digital x-rays (as needed, including full-mouth x-rays and bitewings)
- Oral cancer screening at every exam
- One emergency exam for urgent dental issues during your plan year
- 10% discount on all treatment completed at our practice (excluding Invisalign and teeth whitening)

Membership Details
- Membership is for one year from date of enrollment
- Membership dues are payable in full at the time of enrollment and are non-refundable.
- Services must be completed within the membership year-benefits do not roll over
- This is not dental insurance and cannot be combined with insurance or any other offers.
- Cancellations or no-shows without 48hr notice are subject to a $75 fee.
